The Expat & Immigrant Guide to Swedish Healthcare
How the system actually works: from dialing 1177 and registering at a local Vårdcentral, to specialist referrals, high-cost protection (Högkostnadsskydd), and free treatment under the Communicable Diseases Act.
Core Principles Every New Resident Must Know
- • Decentralized Model: Healthcare is not run by the national government; it is organized by 21 autonomous administrative regions (e.g., Region Stockholm, Region Skåne, Västra Götalandsregionen). Rules and co-pays vary slightly by region.
- • Universal High-Cost Caps: Patient fees are capped. Once you pay 1,400 SEK for outpatient care within a 12-month period, all subsequent visits are 100% free. Prescription medication is capped at 2,850 SEK per year.
- • No Direct Specialist Walk-ins: You cannot walk into a hospital or private specialist clinic without a medical referral (remiss) from a primary care doctor, except in acute life-threatening emergencies.
- • Legal Right to Certified Interpreter: Under the Administrative Procedure Act (Förvaltningslagen) and the Health Care Act (Hälso- och sjukvårdslagen), if you do not speak fluent Swedish, you have the right to a certified interpreter free of charge (0 SEK).
1. 1177 Vårdguiden: Your Entry Point to Healthcare
In Sweden, the telephone number 1177 connects you to a national healthcare advisory service operating 24 hours a day, 365 days a year. When you call, you speak directly with an experienced, registered nurse who assesses symptoms, recommends self-care, or directs you to the appropriate medical facility.
Phone Line (Dial 1177)
Free service from Swedish phones. English-speaking nurses are available. When calling from an international SIM card while in Sweden, dial +46 771 1177 00.
Website (1177.se)
Logged in via Swedish BankID, you can read physician clinical notes, renew prescriptions, reschedule appointments, and view lab results directly.
Emergency vs Triage: For immediate, life-threatening conditions (severe chest pain, acute stroke symptoms, severe breathing difficulty, massive bleeding), never call 1177—always dial 112 immediately for an ambulance.
2. The Vårdcentral (Primary Health Center) & Specialist Care
The Vårdcentral (also known as Hälsocentral or Husläkarmottagning) is the cornerstone of non-emergency medical care. Each clinic is staffed by Specialists in General Practice (Specialist i allmänmedicin), district nurses, physiotherapists, and psychotherapists.
Listing Yourself (Att lista sig)
Under Swedish law, patients have freedom of choice (vårdval). You can register with any licensed healthcare center, whether run publicly by the region or by accredited private healthcare providers operating under the same public fee schedule. You can change your choice up to twice per year via 1177.se.
The Referral Process (Remiss)
Unlike systems where patients self-refer to specialized medical departments, Swedish hospital specialists (cardiologists, neurologists, orthopedists, rheumatologists) require a remiss from a primary physician. The GP performs the baseline diagnostic workup (blood tests, preliminary ECG, standard X-rays). If specialist intervention or surgery is required, they send a formal digital referral.

Swedish healthcare is tax-funded, with modest user co-payments designed to discourage trivial use without creating financial hardship:
| Care Type | Standard Fee (Approx.) | High-Cost Protection (Ceiling) |
|---|---|---|
| Doctor at Vårdcentral | 200 – 300 SEK | Included in 1,400 SEK cap |
| Hospital Specialist (with remiss) | 300 – 400 SEK | Included in 1,400 SEK cap |
| Emergency Room (Akutmottagning) | 300 – 400 SEK | Included in 1,400 SEK cap |
| Inpatient Hospital Bed | ~130 SEK / day | Not capped by 1,400 SEK cap |
| Prescription Pharmaceuticals | Stepwise reimbursement | Maximum 2,850 SEK / year |
| Children & Youths (<18-20 years) | 0 SEK (Free) | Always free outpatient care |
The Frikort (Free Pass): Once your payments reach 1,400 SEK within 12 months from your first visit, you are issued a Frikort. For the remainder of that 12-month period, all outpatient visits to publicly funded clinics and hospital specialists are completely free.
4. Infectious Diseases: 100% Free Care Under Law
Under the Communicable Diseases Act (Smittskyddslagen 2004:168), society assumes full financial and clinical responsibility for controlling dangerous infectious conditions:
Lagstadgade Rättigheter för
klassas som en allmänfarlig sjukdom enligt svensk lag. Det innebär att:
- Helt gratis undersökning & provtagning: Du betalar 0 kr i patientavgift.
5. Your Legal Right to a Medical Interpreter
If you or a family member does not speak fluent Swedish, you are legally entitled to a certified language interpreter (tolk). This right is established in Section 13 of the Swedish Administrative Procedure Act and Section 5 of the Patient Act (Patientlagen 2014:821).
- ✓ Completely Free of Charge: You never pay for the interpreter; the cost is borne entirely by the healthcare region.
- ✓ Strict Confidentiality: Interpreters are bound by absolute professional secrecy (tystnadsplikt) under Swedish law. They are prohibited from disclosing anything said in the room to immigration authorities, police, employers, or family members.
- ✓ How to Request: Simply notify the clinic when booking your appointment: "Jag behöver tolk på arabiska / engelska / spanska" (I need an interpreter in Arabic / English / Spanish).
6. Understanding Waiting Times & The National Care Guarantee (Vårdgarantin)
One of the most common complaints among foreigners in Sweden is the waiting time for non-urgent elective specialist care and surgeries. To regulate this, Sweden established the National Care Guarantee (Vårdgarantin 0-3-90-90):
What happens if the guarantee is breached? If your region cannot offer an appointment within 90 days, they are legally obligated to arrange and pay for your care at another clinic or region, including travel costs.
